Couldn't find it. But to be honest there's plenty of prettier minimalistic skins out there. Try customize.org or deviantart.com. Example: http://customize.org/foobar/skins/56983

i highly discourage using the skin linked by propheticus - it is 5 years old and most certainly relies on outdated plug-ins, for example the description suggests that it's a skin for PUI which isn't working for any foobar release > 1.0 (?)

on the other hand, i've seen skins like this in the DUI gallery thread...
