what's the difference between --alt-preset extreme and --alt-preset 256, wich one is better quality? the 256? or the extreme?

The latter, in most cases. --alt-preset 256 is ABR, --alt-preset extreme is VBR.

Just to make it clear again: --alt-preset extreme will give you higher quality than any ABR preset.
