This has been asked many times before, but I haven't seen a final word on it yet: is it possible to update Foobar without losing anything? And if so: how?
I've read someone that only updated the exe from the zip...but does this really update anything?

That's the way I update. Just replace all files with the newer ones.
I use 7-Zip to open the installer. Then you extract everything(or those you use) in components.
In the icon folder you have foobar2000.exe and shared.dll, extract those too.
Replace the files with the extracted ones. That should be the same thing as install it, except no uninstall function.

As long as you're upgrading from one 0.9.x version to another 0.9.x version and you use the same setting for "Allow per-user settings", your configuration should be kept intact. In fact, I just installed the 0.9.4.3 beta using the installer a few days ago. Since I had enabled "per-user settings" the last time I installed, I made sure to check that option this time, and my settings were all saved.

How do I know what to use then? Can't remember what I chose when I installed it...

Have a look into your fb2k program folder.
If there's a file "user_profiles_enabled", you have "per user settings" (in "%appdata%\foobar2000\"), if the file is not there, you haven't (and fb2k uses it's program directory for config file, playlists etc...).
