Roman Yakimenko, the author of the innovative ScoreExtractor software, has just updated his program. Unfortunately for some, version 2.0 is now shareware... but the author has generously kept the earlier freeware version (1.1) available. From the website:

ScoreExtractor is an interactive multitracks wave to midi converter. The idea is to draw notes on a time-frequency picture, then you can test yourself, and save the results. This process can takes several hours, but the results will be better, than after any automatic conversion. Program can work with uncompressed PCM wave files only.

Depends largely upon the amount of time you are willing to spend refining your conversion, and the quality of instrument banks you have at your disposal. ScoreExtractor is a "visual" MIDI conversion program, which means you should always go through the MIDI output to align each note with its counterpart in the original WAV. And of course, if your WAV includes vocals, MIDI is still unable to replicate the human voice....
