JohnV, I think Mixed stereo mode has nothing to do with mixed blocks.
[Edit : Wooops! I've just realized that the 2nd question didn't have anything to do with stereo modes ]
I've further checked my mp3 collection, and found out that the files that use IS<->Mixed stereo mode are the ones created by
Xing (very old), as reported by EncSpot. (winamp and encspot say they are joint stereo files)
I even have a few that I made myself (from vinyl), but I don't remember which program I used to do them.
(too many years ago.. :·P)

This current sample shows that all frames used are "mixed" (again, in encspot) , although the complete song had a few ( 0.3%!!!!! ) of IS frames.
I was wrong as well with the suposition that "mixed" meant mono, so I don't know what's its meaning.
EDIT 2:
I've also found now a file where the TAG's ( id3v1 and id3v2 ) say that it has been made with
RealJukebox (1), and it has IS-Mixed mode stereo as well.
This is more like a warning, if anyone uses, or have used realjukebox (v1) to encode files to mp3.
